If a fume hood has been red tagged (Exhibit B), an exemption sign posted by EHS (Exhibit A) will be visible on the front of the hood. Compare the sign language to what is actually being done in the chemical fume hood. If they do not match, cite this on the survey as VEN02 and notify the Senior Specialist for follow up.
If the operation that caused the chemical fume hood to be not certifiable has ended (i.e. the large equipment is gone) note this on the survey so that the Specialist can direct BSM to re-certify the chemical fume hood.
If there is a deficiency, Exhibit C shows the BSM identification tag with the number to reference.
