One thing auditors should do unrelated to the checklist is to verify the rooms assigned to the BIO permit. This is not any different than what you do for CHEM permits, it just helps the database accuracy.

Any principal investigator (PI) working with biological materials subject to the UNL Biosafety Guidelines, including recombinant/synthetic nucleic acids (r/s NA), must have a “BIO” permit, assigned by the Biological Safety Officer (BSO). Like the “CHEM” permit, there is specific information such as applicable IBC protocols, list of biological materials used in the lab, and laboratory staff roster. The Permit Type should indicate approved laboratory biosafety level. If it does not indicate BSL-1 or BSL-2, please alert the BSO about the discrepancy prior to scheduling or conducting the audit.

Sometimes rooms listed on a BIO permit are a subset of those listed on the same PI’s CHEM permit. This indicates that only the rooms listed on the BIO permit are approved for work with microorganisms or other biological material (DNA, RNA, etc.). If you find biological materials in a room not listed on the BIO permit, cite it under OTH01.

It lists the NIH Guidelines that apply to the research (you can disregard this), the biological materials in use, the IBC protocol details, rooms, and staff training history. Use the Permit Data Summary document to verify that the facilities they are using are listed correctly in the permit.

To print out a permit data summary, use EHS Assist (EHSA), as with CHEM permits, just select the BIO permit instead of the CHEM permit.
