Other concerns which may require the attention of the BSO.

Use this deficiency to indicate follow-up may be needed from the BSO or another member of the Biosafety team. Examples of observations requiring follow up include:

  • Autoclave performance issues. If the lab occupants indicate any issues with autoclaving of waste, note it here for follow up by the BSO or EHS biosafety staff.
  • Use of respirators, TYVEK coveralls, or additional PPE beyond the requirements or needs of the laboratory. BSL-1 and BSL-2 laboratories generally only require standard laboratory attire and PPE.
  • Occurrence of injuries or illness due to laboratory work (LAI). There is no need to ask about injuries or illnesses, but if the information is volunteered, record it here.
  • Observation of mold in refrigerators, freezers or cold rooms.

If you observe something which may require follow-up, try and gather as much information as possible, avoiding questions which only require “yes” or “no” answers.
